Concenter Realities’ Supportive Living Services (SLS) program offers a flexible, person-centered approach to helping adults with developmental disabilities live independently in homes they own or lease within the community. Guided by each individual’s Individual Program Plan (IPP), these services are tailored to support personal choice, safety, and dignity, empowering participants to shape their own lives while receiving the assistance they need.

Support can include help with selecting and moving into a home, acquiring furniture, and choosing compatible housemates or personal attendants. Once settled, individuals receive ongoing support for daily living activities, such as cooking, hygiene, shopping, managing medications, and responding to emergencies. SLS also encourages full participation in community life, whether that means attending events, accessing local resources, or building meaningful social connections.

Financial management is another key component of SLS. Our team helps individuals learn to budget, pay bills, and maintain financial stability, promoting both independence and peace of mind. 

With a strong focus on self-determination, the Supportive Living Services program is designed to help each person build a life of purpose, stability, and inclusion, on their own terms.

What are Supportive Living Services (SLS)?
Supportive Living Services are individualized services that help adults with developmental disabilities live independently in their own homes or apartments. SLS is guided by the Individual Program Plan (IPP) and includes support with daily living, housing, community participation, and other life skills.

Who is eligible to receive SLS?
To qualify for SLS, individuals must have a developmental disability that originated before age 18 and is expected to continue indefinitely. Eligibility is determined through California’s Regional Center system, and services are tailored through the IPP process based on the person’s specific goals and needs.

What types of assistance are included in SLS?
SLS can include help with finding and moving into a home, choosing housemates and personal attendants, managing money, grocery shopping, preparing meals, attending medical appointments, participating in the community, and responding to emergencies.

Do I need to live in a group home to receive SLS?
No. One of the core principles of SLS is that individuals live in homes they lease or own themselves. SLS supports independent living in the broader community, not institutional or group home settings.

Can I choose my own support staff or attendants?
Yes. A key benefit of SLS is that consumers are empowered to choose their own support staff or attendants, giving them control over who supports them in their daily lives and helping build trusting, respectful relationships.

How is SLS different from other residential services?
Unlike more structured settings like group homes, SLS offers maximum independence and customization. Consumers decide where and with whom they live, and services are delivered based on personal preferences and life goals, fostering dignity, autonomy, and full inclusion in community life.
