At Concenter Realities, we believe every individual deserves the opportunity to live with dignity, independence, and choice. Our Independent Living Services (ILS) are designed to empower adults with developmental disabilities by helping them build the skills and confidence needed to lead more self-sufficient lives within their own homes and communities.

Through personalized support plans developed in collaboration with each participant and their Individual Program Plan (IPP) team, we provide training and assistance in a variety of daily living activities. These may include budgeting and money management, cooking and nutrition, personal hygiene, housekeeping, navigating public transportation, accessing community resources, and more. Our services are rooted in respect and centered on each person’s unique goals, abilities, and lifestyle preferences.

Concenter Realities’ dedicated team of professionals works one-on-one with individuals, reinforcing skills and encouraging progress in a safe, supportive environment. Our aim is not just to assist, but to uplift and enable, so that participants feel empowered to make

decisions, solve problems, and live as independently as possible. Whether someone is transitioning from a family home or moving toward greater autonomy, our ILS program is a trusted partner in that journey.

What is Independent Living Services (ILS)?
ILS is a program designed to support adults with developmental disabilities to live as independently as possible in their own homes or community settings, with tailored assistance in essential life skills like budgeting, cooking, personal care, and community involvement.

Who qualifies for ILS?
To be eligible, an individual must have a developmental disability (onset before age 18) that is expected to continue indefinitely, and which substantially limits daily functioning. The Individual Program Plan (IPP) process helps determine the specific supports needed.

What kinds of support are provided under ILS?
Supports include life skills training (cooking, cleaning, hygiene), financial management, transportation coaching, community involvement, self‑advocacy skills, and coordination with service providers as needed.

Where are services delivered?
Most supports are provided in the person’s home or local community, on‑site coaching in real environments is preferred so that skills generalize to everyday life settings.

How is the level of support determined?
Support levels are set through the IPP process in collaboration with the individual, their family or advocate, and relevant regional center staff. This ensures that services match each person’s goals, preferences, and abilities.

Can someone receiving ILS eventually reduce support?
Yes. Over time, many participants gain skills and confidence. The support structure can be adjusted (reduced, shifted, or customized) to match progress while maintaining safety and independence.

How do I apply for ILS with Concenter Realities?
You or your advocate may request a referral through your Regional Center or contact Concenter Realities directly. After an intake and assessment, we’ll assist with eligibility determination and service planning to begin ILS support.
