Mental health is one of the leading causes of health risks and homelessness in the United States, and its impact is especially severe among individuals with intellectual and developmental disabilities. At Concenter Realities, our Mental Health Network was created to address this critical need by providing timely, accessible, and compassionate care.

Through integrated mental health and psychotherapy services, we ensure that individuals, particularly those with intellectual disabilities, receive the emotional and psychological support they deserve. Our licensed professionals offer personalized counseling that addresses trauma, anxiety, depression, and behavioral challenges, helping clients build emotional resilience and improve their overall quality of life.

What services are offered under the Mental Health Network?
Our Mental Health Network provides counseling, psychotherapy, emotional support, trauma-informed care, and behavioral interventions tailored for individuals with developmental disabilities or complex needs.

Who is eligible to access mental health services?
Individuals with intellectual or developmental disabilities, or co-occurring mental health needs, may be eligible. A diagnosis from a qualified professional and coordination through the Individual Program Plan (IPP) process are required.

How do I start the process to receive mental health support?
Contact Concenter Realities via our enrollment or contact page; we’ll guide you through intake, assessment, and referral to one of our licensed therapists or network partners.

What is the format of therapy sessions?
Depending on the person’s needs and abilities, sessions may be one-on-one, family counseling, telehealth, or in-person, with flexibility in frequency, modality, and duration.

Are mental health services integrated with other supports?
Yes. Our mental health offerings are coordinated with housing, independent living, and supportive living services to provide holistic care and continuity across your support plan.

How is confidentiality maintained?
All mental health services follow clinical ethics and HIPAA (or relevant state privacy law) standards. Information is shared only with consent and on a need-to-know basis among your support team.

Can I switch therapists or adjust my treatment plan if needed?
Absolutely. You have the right to request changes in your therapist or modify goals in your treatment plan as needed. Your comfort and progress are central to the care process.
