Taking this step could be life, changing, it’s the beginning of a path toward greater independence, empowerment, and stability. At Concenter Realities, we are committed to providing supportive services that help individuals with developmental disabilities thrive in their communities. Whether you’re a family member, case worker, or potential consumer, we’re here to guide you through the enrollment process with compassion and clarity.

To be eligible for our services, an individual must have a qualifying developmental disability and have been approved for Regional Center Services before the 18th birthday. These qualifying conditions include intellectual disability, cerebral palsy, epilepsy, autism, and other disabling conditions as outlined in Section 4512(a)(1) of the California Welfare and Institutions Code. We work closely with California Regional Centers and require documentation of eligibility for coordinated support.

Once your eligibility is confirmed, the Regional Center will determine your service catchment area. If you decide we are the best fit, a service coordinator will send us a referral. We will then contact you to schedule an assessment to create your supportive living plan.

From there, our focus turns to a smooth transition into services, whether that’s independent living support, housing, counseling, or daily life skills training. Our goal is to make this process as seamless and empowering as possible, so every individual can begin their next chapter with confidence.

Who is eligible to enroll with Concenter Realities?
To qualify, an individual must have a developmental disability that onset before age 18, is expected to continue indefinitely, and results in substantial limitations in major life activities (such as intellectual disability, cerebral palsy, epilepsy, autism, etc.), as defined under California Welfare & Institutions Code § 4512(a)(1).

How do I begin the application process?
You can start by contacting us directly via phone, email, or the contact form. We’ll schedule an initial consultation to discuss your needs and review eligibility. From there, we guide you through gathering required documentation and submitting a formal application.

What documents do I need to apply?
You will typically need medical, psychological, or diagnostic reports, proof of disability, and any existing Individual Program Plan (IPP). If you are already enrolled with a Regional Center, we’ll coordinate with them for documentation.

How long does the enrollment or review process take?
While timeline may vary by individual circumstances, the review process usually takes 2 to 4 weeks. Delays can occur if documentation is incomplete or further clarifications are needed.

Can family members or legal representatives apply on behalf of someone?
Yes. A consumer’s parent, legal guardian, or designated representative can begin the application on their behalf. All appropriate consents and verification will be required as part of the process.

What happens after my application is approved?
After approval, you’ll work with our team and your Regional Center to develop a personalized service plan. At that point, the selected supports become active, and services begin based on your preferences and goals.
