What services does Concenter Realities offer?

Who is eligible to receive services?
Eligibility is based on having a developmental disability that began before age 18, is expected to continue indefinitely, and results in substantial limitations in daily functioning (per California Welfare & Institutions Code Section 4512(a)(1)).

How do I apply for your services?
You can begin by contacting us through the website or via your Regional Center, completing an intake consultation, and submitting required documentation for eligibility review.

How long does the enrollment process take?
Processing times can vary depending on documentation, coordination with Regional Centers, and assessment needs, but typically ranges from 2 to 4 weeks.

Can someone apply on behalf of a consumer?
Yes. A family member, legal guardian, or designated representative may complete the application process on behalf of a consumer, with proper authorization and consent.

How are service plans created?
After eligibility is confirmed, we work with the consumer, their family or support network, and Regional Center case coordinators to develop a personalized Individual Program Plan (IPP) that specifies goals, supports, and services.

Can I choose my support staff or service providers?
Yes. We value consumer choice, whenever possible, participants can select preferred attendants or staff, housemates, or other providers consistent with their plan.

What if I don’t like the assigned therapist or staff?
You have the right to request changes to therapists, support staff, or service approaches. Your comfort, trust, and outcomes are central to our programs.

Are services available in my area?
Our services may be subject to geographic limitations, licensing, or provider availability. Please contact us or refer to your Regional Center to confirm service availability in your region.

Do you offer remote or telehealth services?
Yes, depending on the service and individual needs, we may offer virtual therapy sessions, remote check-ins, and remote support for life skills or coaching.

How is confidentiality handled?
All services follow clinical privacy standards and applicable state and federal laws. Client information is shared only with consent or on a need‑to‑know basis among authorized providers.

Can services be adjusted over time?
Absolutely. As individuals grow, progress, or face new challenges, service levels, goals, and supports can be revised through the IPP process.

What if my application is denied?
If your application is denied, you will receive a written explanation. You also have the right to appeal or request reconsideration, and you may submit additional information for review.

How are services funded?
Services may be funded through Regional Center vouchers, Medicaid waivers, state or local programs, or private support, depending on eligibility and available funding sources.
