What is Behavior 360?
Behavior 360 is a comprehensive behavioral support program that develops individualized behavior intervention plans (BIPs) to reduce challenging behaviors and promote positive alternatives, within the context of each person’s goals and environment.

Who is eligible for Behavior 360 services?
Individuals with developmental disabilities who display challenging behaviors or emotional regulation needs, and who require structured behavioral support, may qualify. Eligibility is determined through intake assessment and review of the person’s support history, functional needs, and treatment goals.

What kinds of interventions are included?
Services can include functional behavior assessments (FBA), behavior intervention planning, ongoing behavior coaching, crisis support, staff or caregiver training, and monitoring and adjustments to the plan based on data and results.

How are Behavior 360 services delivered?
Behavioral supports may be delivered in the home, in the community, or in other natural settings where behaviors occur. The team works directly with the individual and their support network (family, staff, caregivers) to implement interventions where they matter most.

Can the behavior plan change over time?
Yes. Behavior 360 is dynamic and data-driven, plans are regularly reviewed and modified based on progress, changing needs, or new goals to ensure they remain effective and respectful of the person’s dignity.

Is Behavior 360 integrated with other supports?
Absolutely. Behavior 360 works in coordination with other services like counseling, independent living, housing, and life skills, providing holistic support and ensuring behavioral strategies align with each person’s broader goals.

How do I begin Behavior 360 services with Concenter Realities?
You or your representative can request a referral through our enrollment process or via your Regional Center. After referral, an intake assessment will be conducted and, if appropriate, a personalized behavior support plan will be developed and implemented.
