At Concenter Realities, our Housing & Life Skills program is designed to support individuals with developmental disabilities, particularly older adults or those requiring daily support, in living safely and comfortably within their communities. While some participants reside in assisted living settings, our focus is on helping them maintain as much independence as possible in accessible, apartment-style housing. Each resident lives in their own unit within a larger complex and can choose to prepare meals in their personal kitchen or participate in shared dining experiences when available. This flexible structure promotes both privacy and connection, offering the familiarity of home with the benefit of community-based support.

Our services extend beyond housing to include essential life skills training and non-medical assistance tailored to individual needs. This may include help with bathing, housekeeping, organizing medication schedules, and developing routines for self-care. 

We also provide coaching on practical life skills such as grocery shopping, budgeting, and communicating with landlords or service providers. By integrating housing with life skills development, we empower individuals to lead stable, confident, and engaged lives, ensuring they have the tools and support to thrive in the environments they call home.

What is included in the Housing & Life Skills program?
This service supports individuals by providing assistance with daily living needs such as bathing, cleaning, meal prep, medication reminders, and organizing their living environment in apartment-style settings with optional communal dining.

Who is eligible for Housing & Life Skills services?
To qualify, a person must have a developmental disability that began before age 18 and is expected to continue indefinitely, with needs that require non‑skilled supports to maintain stability and safety in housing.

How is this different from Supported Living or Independent Living Services?
While independent and supported living focus on full autonomy and training, Housing & Life Skills offers more structured support within a housing environment, blending independence with guided daily care and routines.

Where do participants live under this program?
Participants live in accessible apartments or units within a supportive housing complex, with private living spaces and optional communal areas (like dining rooms) to encourage both independence and community interaction.

Can the level of care change over time?
Yes. The support plan is revisited regularly. As a participant gains more skills or stability, the intensity of services can be reduced or adjusted to match evolving needs.

How do I apply for this service?
Start by completing the general enrollment process (eligibility, referral, documentation). If approved, a team will assess whether Housing & Life Skills is the appropriate level of support for your goals and capabilities.

What happens if I need extra support beyond what is offered?
If higher levels of medical or skilled care become necessary, we coordinate with regional centers, clinical providers, and additional services to ensure continuity of care or a transition to a more appropriate setting.
